Hemostatic and soothing effects of oral adhesive bandages in dental extractions

A recent study has shown that the use of oral adhesive bandages were more effective compared to cotton balls and gauze, leading to better hemostatic and comfort outcomes for extraction wounds. This study’s findings were published in the journal, Clinical Oral Investigations.

This randomized controlled clinical study involved 120 patients who were randomly allocated to either the study group or the control group. The control group used gauze and cotton balls, while the study group received oral adhesive bandages for wound dressing. Comfort, hemorrhage, and healing levels were assessed at 1 hour, 24 hours, and 7 days postoperatively. The duration of adhesion for the oral adhesive bandages was also monitored.

The oral adhesive bandages exhibited an average adhesion time of 26.6 hours. The hemostatic levels in the oral adhesive bandage group were significantly higher than those in the control group at both postoperative 1 and 24 hours. Additionally, the oral adhesive bandage group reported significantly higher comfort scores compared to the control group. Both groups demonstrated similar levels of healing and side effects, with a slightly higher mean score for wound healing observed in the oral adhesive bandage group.

The above study demonstrated that oral adhesive bandages are more effective than gauze and cotton balls, resulting in superior hemostatic and comfort outcomes for extraction wounds.

The effectiveness of various in-office treatments for dentin hypersensitivity

In a recent study, all desensitizers that were tested (fluoride varnish (Duraphat - FLU), universal self-etching adhesive (Single Bond Universal - SBU), bioactive ceramic solution (Biosilicate - BIOS), bioactive photoactivated varnish (PRG filler - SPRG) successfully alleviated the initial sensitivity. Biosilicate, the bioactive ceramic solution, demonstrated a gradual decrease in dentin hypersensitivity (DH) after thirty days using computerized analysis. The research findings were published in the Brazilian dental journal.

A total of 192 teeth with exposed roots but no cavities were treated with various desensitizing agents such as fluoride varnish - Duraphat, universal self-etching adhesive - Single Bond Universal, bioactive ceramic solution - Biosilicate, bioactive photoactivated varnish - PRG filler. The level of dentin hypersensitivity was assessed using a visual analog scale (VAS) and computerized visual scale (CoVAS) before treatment, as well as after seven, fifteen, and thirty days first post-treatment session. Statistical analysis was conducted using the Kruskal-Wallis and Dunn's tests, with the Friedman test utilized for comparisons between times (p ≤ 0.05).

FLU exhibited a higher value of DH compared to BIOS at seven days when assessed using VAS, but no significant differences were observed with CoVAS analysis. BIOS and SBU both demonstrated a decrease in DH after seven days, with SBU showing a further reduction at thirty days compared to seven days using VAS. SPRG and FLU groups showed a decrease in DH from fifteen days to thirty days using visual analog scale. Reduction in DH was observed for BIOS,  FLU, and SBU after seven days, while BIOS also showed a reduction at thirty days compared to fifteen days using CoVAS. The SPRG group displayed a reduction from fifteen to thirty days.

The results of the above study showed that all desensitizing agents tested effectively relieved the initial sensitivity. Biosilicate exhibited a progressive reduction in dentin hypersensitivity (DH) over a period of thirty days using computerized analysis.

Pain experienced after endodontic therapy in curved canals prepared with different rotary instrumentation techniques

According to a recent study, pain scores were significantly lower when the XP-endo® Shaper (XPS) system was used compared to the HyFlex® EDM OneFile (HEDM) and WaveOne® Gold (WOG), which showed no variations in pain scores during the follow-up period. The findings of this study were published in the journal Dental and Medical Problems.

Forty-five molars with curved canals and asymptomatic irreversible pulpitis were randomly allocated into three equal groups based on the instrumentation utilized: group A (HEDM), group B (WOG), and group C (XPS). Post-endodontic pain was evaluated using the visual analog scale (VAS) at six, twelve, eighteen, twenty-four, forty-eight, and seventy-two hours after root canal instrumentation. Statistical analysis was performed using one-way analysis of variance (ANOVA) and paired-samples t-test with Bonferroni correction.

Post-endodontic pain reached its highest levels six hours after the treatment. The pain gradually decreased until it nearly disappeared after seventy two hours. There were no statistically significant differences in the VAS scores between groups A and B. Meanwhile, group C consistently displayed the lowest VAS scores at all follow-up time points.

The above study demonstrated that the application of the XPS system resulted in the least amount of pain reported during all follow-up evaluations. No differences in pain scores were observed between the HEDM and WOG groups over the entire follow-up timeframe.

In the first 24 hours, surgical procedures used to speed up tooth movement are linked to a heightened sense of pain when compared with standard orthodontic treatment, but after 7 days, the pain perception is similar. Acceleration surgery is associated with decreased root resorption, especially in alignment and retraction movements. This research findings were published in the journal Dental and Medical Problems.

Data bases including MEDLINE, Scopus, ScienceDirect, Web of Science (WoS), Cochrane Library, and Virtual Health Library (VHL) were searched until September 12, 2022, and included controlled, randomized or non-randomized, parallel-arm or split-mouth clinical trials. Fixed-and random-effects meta-analyses to address heterogeneity were conducted. The risk of bias (RoB) was evaluated using the RoB 2.0 and ROBINS-I tools

Initially, a total of 1,395 articles were collected, with 40 studies meeting the criteria for inclusion in the review, and 15 studies qualifying for quantitative analysis. The meta-analysis demonstrated a significant difference in pain perception between acceleration surgery and conventional orthodontics at 24 hours (p = 0.040). Patients who underwent any acceleration procedure showed significantly reduced resorption rates compared to those treated with conventional methods (p < 0.001). A similar significant difference was observed in retraction movements (p < 0.001) and alignment movements (p = 0.030).

The above study demonstrated that during the initial 24-hour period, surgical techniques aimed at accelerating tooth movement resulted in increased pain levels in comparison to traditional orthodontic methods, but, after a week, the perception of pain is similar.
