Stool withholding behavior is the primary pathophysiological cause of pediatric functional constipation. Stool retention can potentially lead to the development of a megarectum, which in turn can result in reduced defecation sensation and fecal soiling (Figure 1).2
According to pathophysiological studies, withholding behavior is reported in 37%–91% of children with functional constipation, palpable fecal mass on abdominal examination in 33%–68%, and fecal soiling in 33%–77%.2
Passing soft stools without any straining is a crucial aspect in the management of pediatric functional constipation.2 Laxatives are commonly preferred for the treatment of pediatric functional constipation.2,3
A randomized single-blinded clinical trial study by Bastaoros et al revealed that treatment with PEG results in significant improvement in excessive stool retention compared to lactulose.3
